On Mondays, a general practitioner is available at the cancer hub for cancer-related consultations. Appointments can be made if you are concerned about possible symptoms and need screening, or if you require support during or after a cancer diagnosis and treatment.
This service focuses on helping people transition through treatment and back into community-based care, ensuring continuity and confidence at every stage of the journey.
The GP service provides:
-
- Cancer-related medical support and symptom management
- Screening for clients concerned about possible cancer
- Survivorship care planning
- Medicare-enabled coordination (GP Management Plans and Mental Health Care Plans)
- Care navigation and referrals
Having a GP based at the cancer hub means clients have access to trusted medical support in a familiar environment, making it easier to manage ongoing health needs, access care plans, and feel supported at a time when life can feel most uncertain.
Appointments must be for cancer-related consultations and are available for those who haven’t engaged with COUCH, exisiting clients, their families, and carers.
