Working with you to support your patient’s cancer journey
At COUCH Cancer Hub NQ, we work closely with GPs, specialists, and treating teams to support patients throughout their cancer journey. Referrals from a doctor or health professional, allied health providers, or treating teams are welcome but not required. We strongly encourage a shared care approach to ensure each patient receives personalised support and the best possible outcomes. Working together with doctors and health professionals helps us provide coordinated, patient-centred care that addresses each individual’s unique needs.
Our Care Partnership Program is an evidence-based, nurse-led model of care designed to integrate allied health services and complementary therapies. This approach helps patients manage treatment side effects, recover more effectively, and navigate survivorship and palliative care with confidence. By focusing on the patient’s goals and overall well-being, we ensure every individual receives care tailored to their specific situation. The program also encourages collaboration with each patient’s GP, specialists, and other treating teams to maintain continuity of care and ensure optimal health outcomes.
